Chronic diseases are defined broadly as conditions that last 1 year or more and require ongoing medical attention or limit activities of daily living or both. The CDC's National Center for Chronic Disease Prevention and Health Promotion (NCCDPHP) have identified key lifestyle risks for chronic disease including:


  • Tobacco use
  • Poor Nutrition (including diets low in fruits and vegetables and high in sodium and saturated fats)
  • Lack of physical activity
  • Excessive alcohol use

Full-Plate Living Group Program

Designed to help individuals fill their plate for health and weight loss by eating more whole, unprocessed fiber rich foods. Participants will learn 10 tools designed to accomplish long-term weight loss success. They receive a workbook style packet and have the opportunity to taste test fresh fiber packed recipes during class. Classes meet one time per week for 50 minutes each time in-person.
